Deposit
Upon the acceptance of a quote or estimate, a non-refundable deposit of $500 is required to secure the booking of any treatment.
Process
Pure Maintenance of Central Illinois uses a patented two step process for mold remediation. The first step is a dry fog application that is EPA approved to kill mold, called InstaPURE. InstaPURE will fill the home killing all bacteria, viruses and mold that are growing on the surface, behind walls and air born. The second step will be another dry fog application called EverPURE. This will lay an anti-microbial protection against mold, bacteria, virus and other microorganisms on all surfaces of the home. Pure Maintenance of Central Illinois will then take air quality tests to verify a clean environment (when the test is included in the bid). The area will need to be vacant for up to 8 hours from the time of service. Services will be performed at the address listed at the top of this form. Pure Maintenance of Central Illinois will accept cash, check and all major credit cards except American Express. Pure Maintenance of Central Illinois reserves the right to charge 5% interest compounded monthly on all late payments.
Warranty
Mold Services guaranteed for a period of 1 year if and when the entire home is treated. Date one year from the date listed at the top of this form. If at any time during the following 12 months, mold is found in the area of service at a greater density than outside air, Pure Maintenance of Central Illinois will re-treat the home at no charge. Warranty is void if additional water is introduced to premises, i.e. ground water, broken pipe, appliance leak, roof leak, basement flooding, etc. or if any current water issues have not been fixed. Warranty is also void if a current leak or water issue is not fixed before a Pure Maintenance treatment. Any water issues need to be completely repaired and dried out before treatment. If warranty is used, all pages of this bid and invoice must be presented to a Pure Maintenance of Central Illinois employee at the time of a secondary inspection.
Guarantee
Mold Services performed by Pure Maintenance of Central Illinois are guaranteed. After service is completed and before the Pure Maintenance technician leaves, he or she will collect an air sample within the home. The air sample will be taken in the same place as a before treatment sample or if no before treatments samples were taken, the post treatment sample will be taken in a central location. These air samples will be send to an independent 3rd party lab for detailed analysis. Once Pure Maintenance of Central Illinois receives a detailed report from the third party lab, a Pure Maintenance of Central Illinois employee will forward the report to the customer. Pure Maintenance of Central Illinois guarantees that the indoor mold levels will be at safe levels in accordance with current outdoor mold levels.

Photo Release
By employing Pure Maintenance of Central Illinois’s services, you hereby grant permission to Pure Maintenance of Central Illinois to use photographs and/or video of your home taken during inspections and remediation work for documentation purposes, online company profiles, and in other communications related to the mission of Pure Maintenance of Central Illinois. Pure Maintenance of Central Illinois may copy, edit, enhance, crop, or otherwise alter any photographs and/or video for use in their publications. You also waive any rights for approval or inspection of any photographs and/or video. By consenting to the release of images, you agree that you will not receive any form of compensation in cash or in kind. You likewise understand that your name will not be included in the photographs and/or video. Nonetheless, it is still possible that someone may still recognize you. Efforts will be made to avoid personally identifiable information like house numbers and people’s identities. You may rescind your permission to the public release of the photographs by writing us a request and we will make an effort to remove photographs and/or videos within a reasonable timeframe. By employing Pure Maintenance of Central Illinois’s service, you acknowledge you have carefully read and fully understand all the provisions of this Photo Release.
Customer's signature on invoices constitutes acceptance of the following Pure Maintenance of Central Illinois ("PMCI") Terms & Conditions. Final payment is due on the day of treatment. Any invoice amounts remaining unpaid beyond thirty (30) days of invoice will incur a late fee of 5% interest per month, of the original balance, or the maximum amount permitted under applicable law. Customer agrees to pay PMCI's costs of collection of overdue invoices, including reasonable attorneys' fees, court costs, and expenses. Under no circumstances will PMCI be liable to Customer for consequential or special damages. Customer shall indemnify, defend, and hold PMCI harmless from and against any third party claims, demands, lawsuits, damages, liabilities, costs, and expenses, (including reasonable attorneys' fees), incurred by reason of any injury to or any death of any person, or damage to any property, resulting from or arising out of any act, error, omission, negligence, or misconduct by Customer in connection with the services performed hereunder. This invoice, and the Terms & Conditions hereof, shall be governed by and construed in accordance with the laws of the State of Illinois.
